To download:  Right click on the links below and choose "save target as" to save it to your computer.  
Or... just click on the links to open the pdf files and then you can save them from there.

Printing the "HOW TO" Instructions directly from the web page:
Go to the page you want to print (links listed above)
Left click with your mouse (hold it down) and highlight (select) everything you want to print.
DO NOT select the header at the top (ex: Notepad Holders) & it won't  grab the links on the left side.  
(Selecting the links on the left will keep the right side of the page from printing!)
DO NOT select any links at the very bottom of the page either!
After you have selected everything you want to print, go to FILE > PRINT
Choose "SELECTION" under PAGE RANGE
The "SELECTION" option only prints what you highlighted/selected on that page.
If you still have trouble printing the page, change the print orientation from portrait to landscape.
